The recommended dose of LuciDarax(Daraxonrasib) is 300 mg, orally, once daily, until disease progression or unacceptable toxicity. LuciDarax(Daraxonrasib) is available only as 100 mg and 150 mg tablets; select the specific tablet strength(s) yourself based on the actual daily dose.
1. Take at the same time every day: The mean terminal elimination half-life of this drug is 9.2 hours (standard deviation ±2.7 hours). Taking it at a fixed time every day can help keep blood drug concentrations somewhat stable.
2. It may be taken with or without food. After administration with a high-fat, high-calorie meal (800 to 1000 kcal, with fat providing about 50% of energy), no clinically significant differences in the pharmacokinetics of LuciDarax(Daraxonrasib) were observed.
3. Swallow the tablet whole; do not chew, crush, or split it. If the patient has difficulty swallowing, contact the doctor and pharmacist to discuss options rather than crushing the drug yourself.
4. Missed dose management: Take it as soon as you remember. If more than 4 hours have passed since the originally scheduled time, skip this dose and take the next dose at the next regular time. Do not take double the dose at one time to make up for it.
5. Vomiting after taking the dose: If vomiting occurs after taking the dose, do not take an additional dose. Take the next dose at the originally scheduled time. If vomiting occurs repeatedly, especially with difficulty eating or obvious dehydration, contact the treating physician—these are situations in which Grade 3 nausea and vomiting require holding the drug and adjusting the antiemetic regimen.
Dose adjustment of LuciDarax(Daraxonrasib) is not based on personal feelings; the physician determines it based on the actual situation.
Starting dose: 300 mg once daily.
First dose reduction: 200 mg once daily.
Second dose reduction: 150 mg once daily.
Discontinuation limit: If 150 mg once daily is not tolerated, permanently discontinue.
1. Skin reactions, stomatitis, diarrhea: For Grade 2, consider holding the drug and resuming at the original dose or one dose level lower; for Grade 3, hold until recovery to Grade 1 or lower, then resume one dose level lower; for Grade 4, permanently discontinue.
2. Gastrointestinal perforation: For Grade 3, hold the drug, and if appropriate resume one dose level lower; for Grade 4, permanently discontinue.
3. Interstitial lung disease/pneumonitis: For Grade 2, hold the drug, and if appropriate resume one dose level lower; if Grade 2 recurs or Grade 3 to 4 occurs, permanently discontinue.
4. Nausea or vomiting: For Grade 3, resume one dose level lower; for Grade 4, permanently discontinue.
5. Other adverse reactions: For Grade 3, resume one dose level lower; for Grade 4, permanently discontinue.
When LuciDarax(Daraxonrasib) is used with certain drugs, the dose must be adjusted according to the specific situation.
1. With a strong CYP3A inhibitor that also inhibits P-gp: e.g., itraconazole, avoid concomitant use.
2. With a strong CYP3A inhibitor that does not inhibit P-gp: e.g., voriconazole, reduce 300 mg to 150 mg once daily. After stopping that drug for 5 days or 3 to 5 half-lives (whichever is longer), resume the original dose.
3. With a moderate CYP3A inhibitor that inhibits P-gp: e.g., verapamil, reduce 300 mg to 100 mg once daily. After stopping that drug for 3 to 5 half-lives, resume the original dose.
4. With a moderate CYP3A inhibitor that does not inhibit P-gp: e.g., fluconazole, reduce 300 mg to 200 mg once daily. After stopping that drug for 3 to 5 half-lives, resume the original dose.
5. With a P-gp inhibitor: e.g., quinidine, reduce 300 mg to 150 mg once daily. After stopping that drug, resume the original dose.
6. With a strong CYP3A inducer: e.g., rifampin, phenytoin, if unavoidable, increase 300 mg to 400 mg once daily. After stopping that drug for 7 to 14 days, resume the original dose.
7. With a moderate CYP3A inducer: e.g., efavirenz, modafinil, increase to 400 mg once daily. After stopping that drug for 7 to 14 days, resume the original dose.
8. Cyclosporine A and its derivatives (systemic use): avoid simultaneous use.
9. P-gp substrates: e.g., dabigatran etexilate, take at least 4 hours apart from LuciDarax(Daraxonrasib).
Important Note: If a dose of LuciDarax(Daraxonrasib) is missed by more than 4 hours, skip it rather than making it up; do not make up a dose after vomiting. Before starting any new drug (including over-the-counter drugs, vitamins, and herbal medicines), tell your doctor so that interactions can be checked.
