Bempedoic acid is an oral non-statin medication for lowering low-density lipoprotein cholesterol, indicated for adult patients as follows:
To reduce the risk of myocardial infarction and cardiac interventional procedures (such as stent implantation and coronary artery bypass grafting) in patients who are intolerant to recommended statin lipid-lowering drugs or not taking statins, and meet any of the following conditions:
Confirmed coronary heart disease; or no confirmed coronary heart disease but at high risk of cardiovascular disease.
For patients with hypercholesterolemia (primary hyperlipidemia) to reduce low-density lipoprotein cholesterol (LDL-C, "bad cholesterol"), including heterozygous familial hypercholesterolemia (HeFH).
Bempedoic acid should be used in combination with other lipid-lowering drugs on the basis of dietary control; it may be used alone if combination with other lipid-lowering drugs is not feasible.
Bempedoic acid reduces blood low-density lipoprotein cholesterol levels by blocking ATP citrate lyase (ACL), a key enzyme in the hepatic cholesterol synthesis pathway.
