Before initiating treatment with tenofovir alafenamide fumarate tablets, patients should be tested for HIV-1 infection. Tenofovir alafenamide fumarate tablets should not be used alone in patients with HIV-1 infection.
Before initiating or switching to tenofovir alafenamide fumarate tablets, and during treatment with tenofovir alafenamide fumarate tablets on a clinically appropriate schedule, assess serum creatinine, estimated creatinine clearance, urine glucose, and urine protein in all patients. In patients with chronic kidney disease, also assess serum phosphorus.
The recommended dosage for adults and pediatric patients 6 years of age and older weighing at least 25 kg is one 25 mg tablet taken orally once daily with food.
No dosage adjustment of tenofovir alafenamide fumarate tablets is required in patients with estimated creatinine clearance greater than or equal to 15 mL per minute, or in patients with end-stage renal disease (ESRD; estimated creatinine clearance less than 15 mL per minute) receiving chronic hemodialysis. On hemodialysis days, administer tenofovir alafenamide fumarate tablets after completion of hemodialysis treatment.
Tenofovir alafenamide fumarate tablets are not recommended in patients with ESRD not receiving chronic hemodialysis.
No data are available to provide dosage recommendations for pediatric patients with renal impairment.
No dosage adjustment of tenofovir alafenamide fumarate tablets is required in patients with mild hepatic impairment (Child-Pugh Class A). Tenofovir alafenamide fumarate tablets are not recommended in patients with decompensated (Child-Pugh Class B or C) hepatic impairment.
