Mitotane is a medication used to treat specific types of cancer and adrenal gland-related disorders.

What is the recommended dosage of Mitotane?

The clinically recommended dose of Mitotane is usually 6~15mg/kg per day, divided into 3~4 oral doses.

This dose range is calculated based on the patient's body weight and is designed to ensure the effective concentration of the drug in the patient's body while avoiding possible adverse effects of overdose.

It is usually recommended to have the first blood level monitored about 2 weeks after taking the drug. By monitoring blood drug levels, doctors can more accurately understand how well the drug is metabolized in the patient's body, so they can adjust the dose to achieve the best treatment outcome.

Clinical manifestations in patients with adrenocortical carcinoma

Hypercortisolism caused by adrenocortical carcinoma, also known as Cushing syndrome, is a clinical syndrome caused by excessive secretion of glucocorticoids (mainly cortisol) by the adrenal cortex, which is more common in patients with adrenal cortical carcinoma, and its clinical manifestations are complex and diverse, mainly including the following aspects:

1. Changes in posture

Patients often present with a moon face and buffalo back, i.e., significant fat accumulation on the face and trunk and relatively thin limbs. The skin is thin and prone to purple striae, which are often distributed in the lower abdomen, buttocks, inner and outer thighs, upper arms near the armpits, and lower or outer upper breasts of women.

2. Metabolic disorders

Abnormal protein metabolism: due to the action of cortisol, protein breakdown increases, muscle atrophy, and muscle strength decreases. Growth may slow in children, and osteoporosis or even pathologic fractures may occur in adults.

3. Mental and emotional changes

Patients with hypercortisolism caused by adrenocortical carcinoma often have varying degrees of mental and emotional changes, such as irritability, insomnia, dreams, personality changes, depression, and little speech. In severe cases, psychiatric symptoms such as hallucinations and fantasies may occur.

Coping with hypercortisolism due to adrenocortical carcinoma

For hypercortisolism caused by adrenal carcinoma, the main measures include:

1. Medication

Drugs such as glucocorticoid receptor antagonists, ketoconazole, and Mitotane are used to reduce cortisol levels in the body by inhibiting cortisol synthesis or blocking its biological effects.

2. Surgical treatment

Surgical resection of the adrenal tumor is the treatment of choice, and bilateral adrenalectomy plus pituitary radiation therapy may be considered in cases where surgery is not possible.

3. Comprehensive treatment

Combined with chemotherapy, radiotherapy, and molecular targeted therapy to improve the treatment effect. At the same time, patients need to adjust their lifestyle, maintain healthy lifestyle habits, and closely monitor their condition for changes.

【Warm tips】The above measures should be carried out under the guidance of a professional doctor to ensure the effectiveness of the treatment and reduce side effects.
