Recommended Dosage and Administration of Palbociclib

The standard dosing regimen for Palbociclib is 125 mg once daily for three consecutive weeks, followed by one week off (i.e., 21 days on treatment, 7 days off), forming a complete 28-day treatment cycle. You should take the medication at approximately the same time each day, with or without food, swallowing the tablet whole with a full glass of water. Do not chew, crush, or split the tablet, as a broken tablet may lead to abnormal drug release and increase toxicity risk. Your doctor will determine the specific number of cycles based on your condition and response. Please strictly follow the prescription and do not change the dose or stop treatment on your own.

Dose Adjustment Principles for Palbociclib When Side Effects Occur

If certain side effects occur during treatment (especially hematologic toxicity, see article 4), your doctor may take the following measures: temporarily interrupt treatment, reduce the dose, or permanently discontinue the drug. Dose reduction is stepwise, first from 125 mg to the next lower strength of 100 mg; if side effects remain intolerable, it can be further reduced to 75 mg. The dosing schedule (3 weeks on/1 week off) usually remains unchanged after reduction. Additionally, your doctor may delay the start of a treatment cycle based on blood count results. All these adjustments must be performed under close medical supervision. Do not reduce the dose or stop treatment on your own because you feel unwell.

Management of Missed Dose, Overdose, and Discontinuation of Palbociclib

If you forget to take a dose of Palbociclib, or if vomiting occurs after taking the medication, do not make up for it; simply wait for the next scheduled dose and take it normally. Never take a double dose to make up for a missed one, as overdose may cause severe bone marrow suppression and other toxic reactions. If you suspect you have taken an overdose (e.g., swallowing multiple tablets at once), immediately contact your doctor or go to the nearest hospital emergency room, and bring the drug packaging and this leaflet with you so that medical staff can quickly understand the situation. Finally, unless explicitly instructed by your doctor, do not interrupt Palbociclib treatment on your own even if you feel well or experience some discomfort, because continuous, regular treatment is key to controlling cancer progression.
