For constipation, it is recommended to increase water intake, consume high‑fiber foods (whole grains, vegetables, fruits), and use mild laxatives under a physician's guidance; for fatigue, try regular light exercise, meditation, and physical therapy; for edema, adopt a low‑salt diet, elevate the affected limbs, and wear loose clothing; for rash, keep the skin moisturized and avoid scratching. In addition, some patients develop photosensitivity, meaning the skin becomes more sensitive to sunlight and prone to severe sunburn. Therefore, strict sun protection should be maintained during treatment and for one week after discontinuation, using SPF ≥50 sunscreen and protective clothing.

Alectinib may cause relatively serious hepatotoxicity, manifested as elevated liver enzymes, jaundice, dark urine, pruritus, right upper quadrant pain, and abnormal bleeding tendency. Because liver injury is often asymptomatic in its early stages, physicians will monitor liver function at least every 2 weeks during the first 3 months of treatment, then monthly thereafter, with increased frequency based on clinical need. If any of these warning signs occur, you must contact your doctor immediately, and dose adjustment or drug interruption may be necessary. Another serious risk is interstitial lung disease or pulmonary inflammation, which is rare but potentially life‑threatening. Symptoms include new or worsening dyspnea, shortness of breath, persistent cough, and fever. These manifestations can be confused with symptoms of lung cancer itself, so high vigilance is required. If pulmonary inflammation is suspected, the physician usually suspends the drug, performs imaging and respiratory function assessments, and after ruling out infection, may consider corticosteroid therapy and whether to resume the drug.
Both hepatic and pulmonary symptoms require active patient reporting and should not be ignored. Regular monitoring is the key to preventing severe liver injury. Once pulmonary symptoms appear, even if mild, they should be promptly reported to the doctor.
In addition to the liver and lungs, alectinib may affect other vital organs. First, renal injury may present as significantly reduced urine output, darker urine, or new edema in the legs/feet, and severe cases can lead to renal failure; therefore, changes in urinalysis and serum creatinine should be monitored. Second, the drug can induce bradycardia (slow heart rate), causing dizziness, lightheadedness, or even syncope, especially when combined with other heart‑rate‑lowering drugs. Regular heart rate and blood pressure monitoring is needed during treatment. Third, myalgia syndrome is common and may be severe, manifesting as widespread muscle pain, tenderness, and weakness. Physicians will check creatine kinase (CK) levels every 2 weeks during the first month, and if abnormalities occur, dose reduction or interruption is required. Finally, hemolytic anemia, although rare, can occur and leads to premature destruction of red blood cells. Clinically, it may present with jaundice (yellowing of skin), fatigue, shortness of breath, and dizziness. Laboratory tests reveal decreased hemoglobin and positive hemolytic markers. If confirmed, the drug should be suspended, and after recovery, it may be resumed at a lower dose or permanently discontinued.
The above serious side effects are all on the warning list; memorize their early signs. During treatment, avoid concomitant use of other drugs that affect heart rate or renal function without authorization. Regular blood tests are the guarantee of safe medication.
